Billing Inquiries (Including Billing Records Requests)
Effective December 1, 2020, Radiology Associates of the Fox Valley, S.C., and Northeast Wisconsin Vein Center transitioned their billing for professional services to Zotec Partners (Zotec). For billing and medical records inquiries, please refer to the information below:
For patients with dates of services up to and including November 30, 2020:
Please call First Coast Billing Group at (800) 266-9945.
For attorneys requesting itemized billing records, please download the Request for Itemized Billing (PDF) form and fax the completed form to 1 (888) 325-7377.
For patients with dates of services December 1, 2020 and forward:
To speak with one of Zotec’s customer service representatives, please call 1 (866) 863-6739 (7:00 AM to 3:30 PM CST). Please note, if you do not have your account number please enter 0 each time you are prompted for your account number. You will then be transferred to a representative.
To make a payment check, check account balance, request a statement, update insurance information or set up a payment plan, please go to Zotec’s Patient Portal at http://www.mydocbill.com/rria. Patients may also make a payment over the phone at (866) 863‐6740. (Available 24 hours a day, 7 days a week).
Email inquiries may be sent to patients@zotecpartners.com. Please be sure to note in the email that you are a patient of either Radiology Associates of the Fox Valley, S.C. or Northeast Wisconsin Vein Center.
For hospital and/or clinic charges, please contact the billing department at the facility where the services were provided.
